Transitioning Home After a Hospital Stay: Ottawa’s Hospital Discharge and Aftercare Services

Going home after a hospital stay is a major milestone but can come with a mix of relief and what now anxiety. The transition from a clinical bed to your own bedroom requires more than just a ride home; it requires a solid plan.

We’ve walked this path with hundreds of families. We know that caring for a loved one after a surgery or illness isn’t just about following doctors’ orders; it’s about making sure they feel safe, comfortable, and truly supported while they get back on their feet.

The “Home First” Transition

After a stay at The Ottawa Hospital (General or Civic campus) or the Queensway Carleton Hospital, the transition usually happens fast. Ottawa hospitals use a “Home First” approach, which is great because we all heal better at home, but it can feel like a whirlwind.

Before you leave, the hospital team will give you a discharge plan. It’s a lot of paperwork, and it’s okay to feel overwhelmed. Our advice? Take a breath. You don’t have to be the doctor, the nurse, and the housekeeper all at once.

The First 72 Hours: Why They Matter

The first few days at home are the most critical. This is when your loved one is most at risk for a fall or any issues. When we step in to help, we focus on the things that matter most for their comfort:

  • Medication Management: Sorting out the new pills from the old ones so there’s no confusion.
  • Settling In: Getting them tucked into bed, making sure they have water nearby, and checking that the home is clear of tripping hazards.
  • Nutritious Meals: Making sure they’re eating and staying hydrated, which is fuel for recovery.

You’re Not Doing This Alone

Ottawa has an incredible community of professionals who can come right to your door. We often coordinate with local partners to make sure your loved one has a full team who can support them.

  • Nurses: Registered Practical Nurses (RPN) and Registered Nurses (RN) are an essential part of ensuring that your return home is seamless by reviewing discharge instructions and implementing the recovery care at home.
  • In-Home Occupational Therapists: Occupational Therapists can assess and help you make changes to your home or routine to ensure that you are fully supported during your transition home. This will help decrease the risk of falls and/or injury during this crucial
time and regain or maintain your current level of activity.
In-Home Physiotherapists: Staying mobile is key. Physiotherapists can help your loved one regain their strength without you having to drive them to a clinic.
  • Our Compassionate Caregivers: Our team is there for the day-to-day, helping with a shower, preparing a favorite meal, or just providing a friendly face so you can take a much-needed break.
